One of the most common diagnoses with which patients leave the ultrasound office of our office (not including, of course, pregnancy) is uterine fibroids. Many myths and fears are associated with this diagnosis. Someone thinks it doesn't bother me - it's fine, and for someone it's a small fibroid - a disaster. Who is right?

Uterine myoma is a benign tumor of the muscle tissue of the uterus. It is a round muscle formation (node) with a capsule and a vascular plexus, which can be located in any part of the uterus and be of different sizes. There are many causes of this disease, and the most common are genetic predisposition and hormonal disorders. Fibroids grow differently - sometimes very quickly, sometimes - for years. These benign tumors of the uterus can represent a serious obstacle not only to conception but also to carrying the pregnancy to term.

What are the complaints of women with uterine fibroids:
• profuse menstruation that can lead to a decrease in hemoglobin levels and deterioration of the quality of life on "critical" days.
• pain in the lower abdomen
• increased frequency or difficulty urinating
• constipation
• infertility or miscarriage
• abdominal distention
• pain during sexual intercourse.

It is obvious that this disease seriously affects the quality of life, and in the gynecologist's office it is necessary to solve the problem - how to cure it? There are possible options here - surgical and conservative. The choice of method is determined individually, taking into account the age of the patient, future plans for pregnancy, the size and number of nodes, accompanying diseases.

TREATMENT OF MYOMA ON THE UTERUS

In this section, we have also listed the latest methods of treating fibroids, which are not yet widely used and are not available in our country.

1. Hormonal therapy. Until recently, only as a preparation for surgery. Now therapy with regular controls is also possible.
2. Embolization. A minimally invasive procedure, which consists in stopping the flow of blood through the branches of the uterine artery - the artery that feeds myomas. This is one of the most important modern methods of treatment. Unfortunately, it is not available in our country.
3. HIFU (High Intensity Focused Ultrasound) - ablation. Non-invasive procedure. Destruction of the central part of myoma with focused ultrasound. It is also not available in our country.

4. Hysteroscopic resectoscopy. With operative hysteroscopy, submucosal fibroids can be removed through a vaginal approach (without incisions on the abdomen with an extremely fast recovery).

5. Laparoscopic myomectomy. Removal of fibroids with or without suturing by laparoscopic access. Myomectomy involves the removal of fibroids while preserving healthy tissue of the uterus. Laparoscopy is a minimally invasive procedure performed by a gynecologist with the help of special instruments through small incisions in the abdomen. Laparoscopic myomectomy, if possible, is the method of choice, and unlike classic myomectomy, it has fewer postoperative complications and quick recovery. It is very important in younger women as well as in patients who are being treated for sterility due to uterine fibroids.

6. Classic myomectomy. Removal of fibroids by classic surgery with a transverse incision on the front abdominal wall (below bikini level).
7. Hysterectomy. Removal of the uterus together with or without the cervix is performed laparoscopically or classically.
